La présente monographie vise à servir de guide à l'industrie pour la préparation de demandes de licence de mise en marché (DLMM) et d'étiquettes dans le but d'obtenir une autorisation de mise en marché d'un produit de santé naturel. Elle ne vise pas à être une étude approfondie de l'ingrédient médicinal.

- En soumettant une DLMM faisant référence à cette monographie, le demandeur atteste que le produit est entièrement conforme aux conditions d'utilisations recommandées décrites dans cette monographie. Les conditions d'utilisation incluent l'identificateur de la souche de l'espèce de bactéries ou de levures, les quantités d'unités formatrices de colonies (UFC) et l'évaluation complète des propriétés virulentes (incluant mais non limités au profil de résistance aux antibiotiques, à la production de facteurs de virulence et à l'activité toxigène).

3Saccharomyces boulardii Seguela, Bastide & Massot 1984 (Saccharomycetaceae) n'est pas un nom propre valide pour un sous-type génétiquement distinct au sein de l'espèce de Saccharomyces cerevisae (Posteraro et al. 2005). - Consulter un praticien de soins de santé/fournisseur de soins de santé/professionnel de la santé/docteur/médecin avant d'en faire l'usage si vous souffrez de fièvre, de vomissements, de diarrhée sanglante ou de douleurs abdominales intenses (APhA 2006; OMS 2005; CPA 2002).
- Cesser l'utilisation et consulter un praticien de soins de santé/fournisseur de soins de santé/professionnel de la santé/docteur/médecin si des symptômes de troubles digestifs (par ex., diarrhée) se manifestent, s'aggravent et/ou persistent au-delà de 3 jours (APhA 2006; OMS 2005).

Spécifications
L'ingrédient médicinal doit être conforme aux exigences mentionnées dans la BDIPSN et les exigences suivantes doivent être respectées pour tous les microorganismes vivants faisant référence à cette monographie :
- L'identification des espèces par leur nom latin binomial doit être à jour et valide.
- La capacité de survie des microorganismes dans le système gastro-intestinal humain doit être démontrée. Des essais in vitro de résistance à l'acide gastrique et à la bile sont acceptables.
- Les microorganismes doivent être identifiés par phénotype et génotype :
- Le phénotype doit être évalué selon les caractéristiques utilisées habituellement afin de distinguer les espèces entre elles. Ceci inclut une série de tests afin de confirmer adéquatement les traits observables des espèces.
- Le génotype doit être évalué comme suit :
- L'identification des espèces par comparaison de l'homologie de séquence du génome en pourcentage, pour les types de souches 'identiques' et 'reliées de près', obtenues d'une banque de cultures reconnue à l'échelle internationale.
ET - La caractérisation des souches par une méthode de séquence génomique complète et à jour.
- L'absence de virulence de chaque microorganisme vivant doit être établie comme suit :
- La comparaison du profil de résistance antibiotique/antifongique aux espèces typiques résistantes, tel que publié par un panel reconnu internationalement.
ET - L'explication de la base génétique de chacune des espèces démontrant une résistance atypique aux antibiotiques ou aux antifongiques OU la démonstration que tous les mécanismes génétiques de résistance sont absents.
ET - La démonstration que les habiletés de transfert horizontal de résistance aux antibiotiques ou aux antifongiques sont absentes.
ET - La démonstration de la susceptibilité à au moins deux agents antimicrobiens ou antifongiques à des concentrations thérapeutiques et disponibles dans le commerce.
ET - La démonstration de l'absence d'éléments génétiques responsables de la production de facteurs de virulence propre au genre.
ET - La démonstration de l'absence d'activité toxigène (i.e. production de toxines) connu du genre.
En ce qui concerne les informations sur les risques :
- Si des souches de bactéries ou de levures constitutives du produit ont été en contact avec des allergènes prioritaires ou avec un de leurs produits dérivés (par ex., le soya, le gluten, le lait ou le poisson, par l'entremise du milieu de culture) (liste disponible à : http://www.hc-sc.gc.ca/fn-an/securit/allerg/fa-aa/index-fra.php) et que ces allergènes ne sont pas mentionnés sur la liste des ingrédients médicinaux ou non-médicinaux, un des énoncés de risques suivant doit être inscrit sur l'étiquette du produit :
- Ne pas utiliser ce produit si vous êtes allergique à XXX (CG 2011; SC 2009);
OU - (Peut) contenir XXX (SC 2012a; SC 2012b; CG 2011; SC2009; SC 2003).
- Si des souches de bactéries ou de levures faisant parties du produit présentent une résistance atypique inexpliquée à un ou plusieurs antibiotiques (Mathur et Singh 2005), le(s) nom(s) de(s) l'antibiotique ou des antibiotiques doit/doivent être indiqué(s) sur le formulaire de DLMM et sur l'étiquette en tant que contre-indication de la façon suivante :
- Ne pas utiliser ce produit si vous prenez XXX (par ex., Ne pas utiliser ce produit si vous prenez de l'ampicilline).
Nota:
L'énoncé de risque ci-dessus ne s'applique pas aux demande de classe I (officinal) et de classe II. Si applicable à la souche attestant de cette monographie, des preuves supplémentaires doivent être fournies et examinées dans le cadre d'une évaluation de classe III.
Les spécifications du produit fini doivent être établies conformément aux exigences décrites dans le Guide de référence sur la qualité des produits de santé naturels de la Direction des produits de santé naturels et sans ordonnance (DPSNSO). Dans le cas des microorganismes vivants, ceci inclut les exigences suivantes :
- Les mesures de stabilité/viabilité mises en place doivent assurer qu'un minimum de 80% de la quantité déclarée sur l'étiquette du produit est présente jusqu'à la fin de la durée du produit.
